St. Charles County Dissolution of Marriage Form – Respondent
The person who is being served divorce papers is called the Respondent.
The Respondent must file the Respondent’s Answer form and Statement of Income and Expenses, Statement of Property and Debt, and Parenting Plan, if applicable.
These papers are also available through the Missouri Courts website (click here). You can fill them out online before printing, or download the blank form into your computer.
St. Charles County Motion for Visitation Form
If there is an existing court order on child custody or visitation but you believe that it has been violated, you may seek the court’s help to enforce it.
To do this, you can file a Motion for Family Access Order. The form is available here.
If you are the party being accused of violating the child custody or visitation order, file your response via this form.
St. Charles County Petition for Child Custody and Support Form
Even if you’ve never been married to the other parent of your child, you may request the court for a child custody or support order as long as the paternity has been legally established. The packet of forms for this petition can be found here. It includes, among others, your main Petition paper, proposed Parenting Plan, and statements of income and property.
If you are the non-filing parent in this case, you will need to respond to the petition using these forms.

To ask the court to modify your existing child custody or support order, you must accomplish this packet that includes your Motion to Modify Child Custody and Support, proposed Parenting Plan, Statement of Income and Expenses, and Statement of Property and Debt, among others.

Mother as Petitioner for Paternity
If you are a mother who wishes to legally establish that a man is the father of your child (or children), you may petition the court to do so. The packet of forms can be found here, which includes your proposed Parenting Plan and a form for the appointment of the “Next Friend,” the individual who may act on behalf of your minor child.
If you are the man being claimed as the father in this case, you must respond to the petition using these forms.
Father as Petitioner for Paternity
On the other hand, if you are a dad who seeks to legally establish your fatherhood of a child, you may file your petition with the court using this packet of forms. It likewise includes papers for your Parenting Plan and appointment of the “Next Friend.”
If you are the child’s mother in this case, you must respond to the petition using these forms.
Petitioning for Non-Paternity
A man can be “presumed” to be the father of a child if this man is married to the child’s mom. If you are the presumed father of a child but wish to declare that you are not their legal father, you may ask the court to declare this. The forms you need to accomplish are in this packet.
If you are the mother of the child in this case, you must respond using these forms.
St. Charles County Petition for Change of Name Form
You may ask the court to legally change any part of your name. There are separate form packets for changing your name as an adult and as a minor, and they can both be found here. Minors need the written, notarized consent of their parents before they can proceed with their name change.
A whole family may also request for name change together. They will need to accomplish the forms above for adult and minor family members, then file these together as a joint petition.
